2024-25 Travel Permit Request (Milton Minor Hockey)

Rep and Select Teams are permitted a MAX of 3 exhibition games a month based on VP rep and OMHA approval;

House teams must get prior approval from VP House for exhibition games and  to enter tournaments, 

Stipulations:
      * Exhibition games/tournaments are NOT allowed prior to having an OMHA approved roster-request from                 MMHA office
      * requests may not be approved if ex game or tourney interferes with regular season schedule
      * Depending on ice availability, teams may not have missed practices rebooked
      * Approval of travel permit from the OMHA can take at least 7 days..
      * Submission of this form does not mean you are approved for your permit
      * Once approved by OMHA, permit will be emailed to you by OMHA (Contact Management). 
      * Travel permits are required for ALL exhibition games/tournaments regardless of location (home or away)
      * Referees require a minimum of 72 hours to be scheduled for exhibition games.  Once the game goes on your calendar, the referees will be notified to schedule.
      * Teams are responsible for finding their own timekeepers for home exhibition games.
      * Teams will be invoiced for travel permits for all  tournaments outside the OMHA (i.e. Alliance or GTHL or out of province) and all exhibition games outside of OHF. For Houseleague: Request dollar amount from office so team can collect.
       *Teams are responsible to abide by the OMHA allotted number of ex games/tournaments per team  
       

I acknowledge that I have provided our team with Hockey Canada insurance information if travelling out of province/country. We are responsible for obtaining appropriate insurance as required.
