The team trainer is responsible for requesting a first aid kit, and with returning the kit to the office at the end of the season.
Failure to return the kit by March 25th of each season will result in a monetary penalty of $30.00 being charged to the team trainer. Please contact the office if supplies need to be replaced thru the season.
Once the online form is complete, please pop into the office to pick up your kit.
