MMHA Complaint Form
Complaints must be filed within 30 days of the alleged incident(s). Anonymous complaints will not be investigated. Complaints will be addressed in accordance with the Code of Conduct Policies and Procedures.

Nature Of The Complaint

If this matter has been reported to the Police or Child Protection Authorities, the Milton Winterhawks / OMHA may through its fact-finding process determine that a suspension of the alleged offender is warranted, until such time as the Police and/or Authorities have concluded their investigation, after which a final determination will be made by a disciplinary panel. Complaints of misconduct will follow the Milton Winterhawks Dispute resolution process.
